Verification of Work Experience


Work experience is a requirement for certain certificate applications. Information regarding the experience requirements can be found here: Experience Requirements 

All work experience must be verified by the employer using the specific instructions for each section below. Some experience must be verified via Superintendent Statement in TEACH, and some can be submitted using the forms linked below.

Notes on New York City Department of Education (NYCDOE) experience
  • Teaching and Pupil Personnel Services experience: Superintendent Statements must be entered by NYCDOE Human Resources Directors.

  • Teaching Assistant experience: Superintendent Statements must be entered by NYCDOE Office of HR School Support - Paraprofessional Milestone Unit.

  • Administrator experience: Superintendent Statements must be entered by NYCDOE Supervisory Staffing Unit.

Permanent/Professional certificates & Teaching Assistant Level 2 & 3 certification

For these types of applications, New York State public school districts, BOCES and charter schools located in New York must verify paid work experience by entering an electronic Superintendent Statement onto the applicant’s TEACH account. Approved nonpublic schools located in New York (independent schools, private special education schools, early childhood centers, etc.) that have administrative access in TEACH can enter Superintendent Statements as well.

The forms in this section below can only be submitted by approved private schools located in NY that do not have administrative access and schools located outside NY.


Out-of-State applicants

The forms in this section below are for applicants pursuing certification using certified public school experience and evaluation ratings completed outside New York State for the “Endorsement” pathway.

Student Teaching/Practicum requirements

This form is strictly for applicants using paid teaching experience to satisfy either the college-supervised student teaching or practicum requirement.


Career and Technical Education (CTE) Special Education Teachers only

The forms in this section pertain to specific special education teaching experience completed in New York State.
