Paid Experience Verification Form for Employers


1. All experience must be verified by your employer.

  • New York City Department of Education (NYC DOE) Employees

    NYC DOE Teaching Assistant experience must be verified by:

    • NYC DOE Office of HR School Support, Paraprofessional Milestone Unit

    NYC DOE Teaching Experience must be verified by one of the following:

    • NYC DOE Human Resource Director; or
    • NYC DOE Community Superintendents.

    NYC DOE School Building Leader and School Administrator experience must be verified by:

    • NYCDOE Supervisory Staffing Unit

  • For All Other Employees
    Experience must be verified by one of the following:
    • Superintendent;
    • Superintendent’s Designee;
    • Head of Human Resources; or
    • Chief School Officer of the Approved Nonpublic/Independent School.

2. The submission instructions are located on each form.

Before printing one of the forms below, contact your employer to determine if your employer will submit a Superintendent Statement directly in your TEACH account verifying your experience.
