Program Registration Applications, Guidelines, and Resources

This page contains links to the forms needed to register a new program below the doctoral level or to change an existing program. It also links to related guidelines and resources. Please note the following important exceptions:

  1. Any institution considering a new doctoral program should contact the Office of College and University Evaluation at (518) 474-1551 for the document, "Steps in the Review of Doctoral Program Proposals."

  2. Public institutions should use the appropriate SUNY/CUNY proposal materials, in consultation with the central administration of the system and according to established procedure.

  3. Programs intended to prepare individuals for licensure in a profession licensed by the State Education Department will be evaluated by the Department’s Professional Education Program Review office, in the Office of the Professions. Please review the guidance on this page about professional licensure programs.

Program Registration Applications by Type of Proposed Program

General Academic Programs
Use these forms for general academic programs; such programs are not registered as preparing individuals for a teaching certificate, educational leadership certificate, or a professional license issued by the State Education Department.

Programs to Prepare Candidates for a State Professional License

Programs intended to prepare individuals for licensure in a profession licensed by the State Education Department will be evaluated by the Department’s Professional Education Program Review office, in the Office of the Professions. Before preparing an application for a professional licensure program, contact that office for more information about additional materials that may be needed to supplement the core program registration form.
