Name Change
There is no fee for requesting a name change.- Log in to your TEACH account: Login to TEACH
- Click "Update/Add Education, Employment, and Personal Information".
- Click “Edit Personal Info”.
- Click the “Request Name Change” button.
- Enter your name as it is listed on your Government issued photo ID.
- Upload a copy of the supporting documentation. Acceptable documentation includes:
- Government issued photo ID such as a driver’s license, driver’s permit, or non-driver ID card.
- Government issued passport.
- Government issued certificate of naturalization.
- Click "Submit" at the bottom to send your request.
**Supporting documents must be in .JPG or .PDF format. We cannot open compressed files (example: .HEIC, .ZIP), or cloud files (example: Dropbox, iCloud, Google Drive)**
