Superintendent Verification of Mentored Experience

The Superintendent Verification of Mentored Experience form may be used for all Initial certificate holders employed in the district as they complete the district’s planned mentored experience or receive a waiver to this certification requirement in accord with Commissioner’s Regulations. It is suggested that one copy of the completed form be retained in district files and a copy be provided to the certificate holder.

The certificate holder should submit his or her copy of this form either at the timme of completion of the mentored experience or at the time of application for the Professional certificate to:

Availability of superintendent verification of the mentored experience on TEACH

This aspect of the TEACH system is due to be constructed. At this time, the capability to verify completion or waiver of this certification requirement is not available on TEACH. However, we do expect this feature to be built into TEACH in the near future.
