Limited Certificate

Does a person who is issued a “Limited” certificate for 2003-2004
need to submit a separate application at the end of that year in order to
be issued a Provisional certificate?
Yes. The application for certificate is available at
Applications and Forms. Holders of Limited
Certificate must meet the requirements and apply for the Provisional certificate
no later than 8/31/04.
