Submitting Transcripts

Read the scenarios in the table below to determine if you need to submit transcripts. If you are required to submit transcripts, you must have Official Transcript(s) – in sealed envelope(s) – sent directly to this office by each of the institutions you attended. If coursework appears as transfer credit, an official transcript showing the coursework must be submitted from the college at which the courses were completed.

All institutions that you attended must be identified in your TEACH account in the “Education Information table” when you apply for a certificate in TEACH. You may also update this information by going to the link: “Update/Add Education, Employment and Personal Information” in your TEACH account.

Official transcripts must be mailed from the institution to:

Photocopies, faxes, student copies, grade reports and digital (non-paper) transcripts are unacceptable. Each transcript must contain your identifying information:

  • date of birth or a portion of your date of birth
  • the last four digits of your social security number
  • your current name exactly as it is listed in your TEACH account.

    • Ask the registrar if your institution includes your date of birth or a portion of your date of birth, and the last four digits of your social security number on official transcripts.  If your institution does not include this information, you must have the institution include this information in a letter attached to your transcript.
    • Transcripts received without this information cannot be matched and entered into the “Verified Education” table of your TEACH account and therefore cannot be used in an evaluation of your application.

    • If you requested a transcript to be sent to the Office of Teaching Initiatives by your Institution and it is not recorded in your TEACH account within four weeks it may be that your identifying information was not included on your transcript. Ask your Institution to send a new official transcript with your identifying information included.

I have never applied before and I am applying under the Individual Evaluation pathway.

Official transcripts are required from every institution you attended (Information about transfer credit).

I have never applied before and I have completed a New York State approved teacher preparation program for this certificate and am applying via the NYS “approved teacher  preparation program” pathway.

NO

NOT for this certificate application

Your NYS college should submit and electronic ‘recommendation’ to your TEACH account which verifies that you have completed an entire teacher, pupil personnel or leadership certification program.

I have never applied before and I am applying under the Completion Of A College Teacher Education Program In Another U.S. State or Endorsement Of A Classroom Teaching Certificate pathway. I am certified with a level 2 or higher certificate from another state and have three years of experience in that state under the certificate.

YES

In addition to sending your transcripts, send a copy of your certificate(s) and enter all of your Teaching experience in your TEACH account.

I have never applied before and I am applying under the Endorsement Of A Classroom Teaching Certificate pathway. I am certified with a level 2 or higher certificate and do not have three years of experience under the certificate.

YES

Scenario 2

Do I Need to submit transcripts?

I already hold a certificate or have been evaluated for a different certificate

I previously applied for another certificate via NYS approved teacher preparation program pathway, and I am now applying for an additional certificate via transcript evaluation.

 

YES

Official transcripts were not required under the NYS Approved Program pathway. Your application will not be “ready for review” until you send official, acceptable transcripts.

I am applying for an additional certificate and I applied for my other certificate(s) under the transcript evaluation pathway.

If your transcripts were submitted more than three years ago, you will need to have official transcripts sent. Transcripts are securely destroyed after three years.

 Check the “Date Received Column”* in the “Documents you sent to OT/BOCES” table in your TEACH ONLINE ACCOUNT (see example below). If the Date received is more than three years or there is no date you will need to submit new Official transcripts.
