Duplicate Certificates

It is the certificate holder's responsibility to safe-guard his/her certificate as one would any important document. School district Offices of Human Resources may require that individuals show proof of certification in a specific area as a condition of employment.

Duplicate Copy of Issued Credential(s)

Individuals holding a certificate that has been lost or destroyed may apply for a duplicate copy of the certificate(s).

Individuals who have not kept their address up to date with this office, and as a result do not recieve thier certificate, will be required to pay a $25.00 fee for a duplicate certificate.

  • Submitted Online:  Click Indicates you can Apply Online to apply for a duplicate certificate.  The fee is $25, which can be paid by credit card.  Once your application and payment are submitted your duplicate certificate will be mailed.

Verification of New York State Certification

The Office of Teaching Initiatives does not provide letters verifying an individual's New York State teacher certification status. If verification of a New York State credential is required for certification outside of New York State, each state teacher-certifying agency may obtain this verification through access to the certification files via our Indicates you can Apply Online.
