Yes, it has been at least 18 weeks since I submitted my application.

First check to see if your certificate has been issued.

If your application status in TEACH is “Review Complete - Pending Information” and you received a notice of unmet requirements:

1. Check your records. Have you submitted all outstanding requirements according to your last evaluation or correspondence from us?
   
   When possible, send all missing components at the same time, in the same package. Requirements sent in one-at-a-time cause unnecessary delays. (Note: Test scores are sent to NYSED directly by National Evaluation Systems, NES.)  Evaluations are completed only through the mail or online.
    
2. If you have questions about your evaluation, submit them in writing by regular mail to the Office of Teaching Initiatives.   Please include your full name, social security number, a copy of the evaluation and any necessary supporting materials.  An example of supporting material is a catalog course description from the college.
   
   Mail everything in one package to:

3. Any additional support materials you send to us will be added to your file.  Your file will go back in line for review.  All files are reviewed in the order they are received.
