You must submit an application for certification:

For any new certificate, a completed application with appropriate documentation and fee must be filed with the Office of Teaching Initiatives.

Additional Information

NOTE: PAPER RECORDS ARE DESTROYED AFTER THREE YEARS. You must submit original transcripts from all colleges/universities attended with grades, GPA, and, if applicable, the conferral date(s) of the degree(s) awarded if you have not done so within the last three years.

Fingerprint clearance, proof of completion of the Child Abuse Identification and Reporting Workshop and  proof of completion of the School Violence Prevention and Intervention Workshop are one-time submissions.  These, along with scores of NYSTCE exams as reported to NYSED by NES, make up an applicant's computer file and do not need to be resubmitted with each additional application for certificate.
