DIRECTORY OF OFF-CAMPUS INSTRUCTIONAL LOCATIONS

Check for missing, invalid, out-of-range, or duplicate institutional codes and record identifiers. Check for missing forms. This form applies to all locations of all degree-credit institutions.

• Location Data:

1. Box checked indicating that the institution has a type of off-site instructional location: the corresponding part of the form should include at least one location name.
2. If no box is checked, then the institution has no off-campus instructional locations.

Fatal error checks 

• Name & Address – if not all blank, must have name, address line 1, city, zip, state or country

Range Checks -

• Branches: No. of Degree Programs – must be between 1 and 50 for each reported branch
• Extension Centers: Number of courses must be >0 and < 100 for each reported extension center
• Extension Centers: Course Registrations – must be >=1 and < 1000 for each reported extension center
• Extension Centers: No. of Courses must be >15 or No. of Course Registrations must be >350 for each reported extension center
• Extension Sites: Number of courses must be between 1 and 15 for each reported extension site
• Extension Sites:  Course Registrations – must be between 1 and 350 for each reported extension site

Comparison error checks.  

• None

• Counts for each type of off-campus instructional site as follows:

• Number of programs (branches) or courses (extension centers and sites)
• Number of course registrations (extension centers and sites)

These counts should be created for:

• Branch campuses (including interinstitutional programs)
• Extension centers (including interinstitutional centers)
• Extension sites (interinstitutional sites)

Off-campus locations include:

• Sites within and outside New York State
• Sites providing special facilities
• Secondary school sites
• Correctional facility sites
• Native American reservation sites
• Clinical/practice and other sites providing exceptional services
