Description of each type:

The program has never been offered before; or

There has been a period of time during which a previously approved curriculum was no longer valid.

The program is currently approved, but its expiration date is approaching and the school wishes to continue the program (with or without changes)

The program is currently approved, is not near its expiration date, but change is desired.

The school submits (in duplicate):

All sections of the curriculum application must be completed with the exception of items 5, 7, and 14.

Completed curriculum application cover page, items 1 through 18, and any additional items which would be affected by changes identified in Item 14.

Curriculum application with items 1-7 and 14 - 16 completed. Additionally, any items that correspond to the changes desired must also be completed.

Effect upon approval:

The school has a new program to offer in addition to those currently approved.

The previously approved curriculum approval is continued to a new, extended expiration date. Any changes are effective for program starts on or after the effective date of the reapproval.

The changes listed in the amended curriculum are effective for program starts on or after the effective date of the amendment approval.

Curriculum ID Number:

The new program has a new and unique curriculum ID number.

The reapproved curriculum has the same curriculum ID number as the previous curriculum.

The amended curriculum has the same curriculum ID number as the previous curriculum that it replaces.
